Referring offices can either use the "Digital Referral Form" to securely send patient information and radiographs OR download the "Printable Referral Form" to send your patients to Idaho Emergency Dental. Complete the form and have your patient bring it to their appointment, or send it to info@idahoemergencydental.com.

Take the Eagle Exit to E Overland Road to find our clinic. From the parking lot off of E Gala St, you will see the entrance pictured below; then follow signage to SmileWorks clinic.
